Overview
We are seeking a dedicated and compassionate Primary Care Nurse Practitioner/Physician Assistant to join our healthcare team. In this role, you will provide comprehensive care to patients, focusing on preventive health measures and managing chronic conditions. Responsibilities

• Conduct thorough patient assessments, including medical history and physical examinations.
• Develop and implement individualized care plans based on patient needs.
• Perform diagnostic tests and interpret results to guide treatment decisions.
• Administer treatments, including IV insertion and tube feeding, as necessary.
• Manage patient care in various settings, including memory care facilities and teaching hospitals.
• Participate in discharge planning to ensure smooth transitions of care for patients.
• Maintain accurate medical records and documentation for all patient interactions.
• Engage in utilization review processes to optimize patient outcomes and resource use.
• Provide education and support to patients and their families regarding health management.

Skills

• Strong clinical skills in internal medicine and primary care practices.
• Proficient in venipuncture techniques and other essential nursing procedures.
• Excellent communication skills for effective patient interaction and education.
• Ability to work collaboratively within a multidisciplinary team environment.
• Knowledge of discharge planning processes and memory care considerations.
• Familiarity with medical record management systems.
• Strong organizational skills with attention to detail in patient documentation.

Job Type: Full-time

Pay: $90,000.00 - $99,999.00 per year
